Apply for long-term care indication (Wlz indication)
If you need intensive long-term care, for example, due to a disability or mental illness, you can receive care under the Long-Term Care Act (Wlz). For this, you must request a Wlz-indicatie (Wlz indication) from the Care Needs Assessment Centre (CIZ). The CIZ will assess the care you need.

Wlz indication conditions
‘Wlz’ is the Dutch abbreviation for the Long-Term Care Act. With a Wlz indication, you can receive care under this act (in Dutch). You can then receive a place in a long-term care home or other care institution. Or you can receive intensive care at your home.
To get a Wlz indication, you must meet certain conditions. For instance, you must require access to care close at hand 24 hours a day. The conditions (in Dutch) are available on the CIZ website.
