Declaration of relationship for COVID-19 entry ban exemption

The Dutch govenrment has decided to ease the travel ban on the entry of partners from third countries engaged in a long-distance relationship with someone from the Netherlands, subject to certain conditions. The rules will come into force on 27 July 2020. Only complete this declaration if you want to travel to the Netherlands for a short stay.
